Sometimes, certain zombies, like "big mamma", will keep whiffing attacks on you if you look at them *dead on". This is when they're right in front of your face. You're basically invincible as long as you position the camera directly at them. However, if you turn ever so slightly, they'll manage to connect their attacks 100% of time. This bug is inconsistent, happening in about 40% of frontal attacks. Big mamma is the worst offender in my tests, especially her right-handed attack.
