Suggest to add the radiation 1-100% level sumular like infection which will grow as soon as u will be near radiated zombies or radiated world objects like rad barrels! To lower radiation level, u will need to consume antirad pills (the rad healing would work same as infection cure state)
The radiation disease power would depend on a stage: from weak stage 1 to strong stage 3 with the debuffs of HP, speed, strenght and etc!
Also suggest to rework player smell in to same 1-100%, it will grow slowly over time, searching for loot and etc! To cure smell u would need to take a shower with soap (not just going in to the water for 2 sec)! In addition strong bad smell would cause some discomfort for player
