Does it roll every time you perform an action...like loot, shoot, etc?  Or is there a roll for x item when the game is generated?

'Cause on this play through I found a grilled meat schematic.......which I have not found before...but now I cant find a darn grill.  

I know, same old story, is just weird.  Last night I went to the local appliance store.  Fought off a zillion Zs (I swear I think I found the mini horde spawn point on this map :)  j/k).  Looted all appliances, some nice 70s vintage stoves, not a single grill heh.  I am sure I will find one in a mail box somewhere heh /sarcasm.

 
It makes the rolls when you open a container, not before. You can watch it in action with the Loot Logger mod (including seeing the actual odds of getting a grill in each container you loot) if you want.

 
I have had the same thoughts but chock it up to confirmation bias.

But yes it seems some items are consistently more rare than others in some play throughs.

As for the grill, if I don't loot a cooking pot day on one, I bite the bullet and spend a point for the forge (for the pot and grill)...
