Working stiff and construction supplies have highest drop rates. But it's really low. On this play through (day 38) I haven't found one yet. But 3 previous play throughs (day ~21) had at least one every time.
Bahroo tried fire trap yesterday on stream. Worked pretty good.
Eat blue ice. Can be found in medical boxes and can be created by medic class.
there was an update to lower the spawn rate of the diner towers, it did help cuz that building is annoying as hell and was in every single city and town in previous iterations.
I found 2 working stiffs stores (big 1-story type) in one town right next to each other.
I download manually and with the launcher the last version and try 3 diferents plays in bot systems and i am very sure the dinner tower is not patched, 90% to 97% of cities has a dinner tower and for make sure i active the debug mod and teleport to diferents points and the tower is in almost all cities.

Had this way back when in vanilla on a rented server. Ground disapppeared, same kinda things and as I recall it was due to my system getting hot and / or running out of resources. That system had less memory than my current one but I never could place the blame.
was this the big giant hospital? if so that was removed for version 3.1 (needed a full wipe from 3.0)
if not that, sounds like a terrible world gen glitch but really not sure.
also happy you didn't lose anything, do you back up your save game files?
located somewhere like
C:\Users\name\AppData\Roaming\7DaysToDie
you would create an empty folder somewhere else, then copy that 7DaysToDie folder into it.
that way if your world borks (modded, vanilla, whatever) you can pull it fresh from there.
