Windows Quest flags on POI with no quest?

A few times now I have come across a POI that had a quest flag, but my only quest was at a different POI, e.g. I had a tier 1 quest set active but came across a tier 4 POI with a flag. I could not interact with the flag in any way.

I looked through the Bug report forum but didn't see anything similar unless I missed it. Is this a known issue, or some feature I haven't figured out?
 
The only time I have come across this was in internal testing when there were changes to the map though a save.

Unfortunately, with such limited information I cannot provide you with further information.

Would need screenshots of your quest with its location information, a screenshot taken using the console command "dbs" at the location where you are seeing the flag, and the client logs from when you had this encounter.

If you are playing on a world someone else is hosting, or are playing on a dedicated server, we will need logs from the host client as well.
 
The flags I saw before are gone, but I can find plenty of new ones now. I guess my save is getting corrupted.

I took another quest and went to the location. I looked around a bit and saw a ghost flag. Took a screenshot, and then noticed another flag at another POI. Long story short, flew around in god mode and flags were appearing at many of the POIs in the town. Sometimes they would not appear but show up again when I flew back. I flew to another town and saw some flags there as well. I am using a few simple UI mods (HP bars, more visible food and water) that I doubt are causing this but I guess I can't rule them out. Maybe I'll remove the mods and see what happens.

Just remember that removing a mod mid-game can cause problems as well. A true test would be to remove them and then play a new save and see if you get the same results. In either case, a lot would help people to look into it.
