PC Near death?

Had something strange happen yesterday and it is bugging me. I was exploring a house stealthily when something exploded nearby (cop zombie?). My screen froze and a good chunk of the house was missing. I then got the option to respawn. I respawned and had the "Near Death Experience" indicator, but my deaths count was still zero. What gives?

Okay that's a weird one.

Might have been a car that exploded?

Still doesn't explain all the rest.

That's just weird.
