PC Near death trauma - after first death?

OK I get that you should get a debuff if you die multiple times in short span of time. But I just got debuff on day 11, never died before and today while mining whole thing collapsed on me and ofcorse as mentioned I died for the first time.

But it was to my understanding that debuff is only a thing on repeated deaths and not after single death or am I getting this debuff description wrong?

 
You were mistaken. It's after any death.

You do have a grace period when you first start. I don't know exactly how long but if you die right after starting you won't get the penalty.
