Dedicated Server Death Penalty 1 (Traditional XP) is triggering 2 (Ailments) on respawn in some instances

A Dedicated Server used in console compliant mode has the DeathPenalty value as 1 (Default: Classic XP Penalty) but is triggering 2 (Injured: You keep most of your debuffs. Food and Water is set to 50% on respawn.) in about 20 - 30% of cases upon respawn based on feedback. Since this server is using Insane difficulty, players are able to provide a lot of feedback on this discrepancy due to a higher death rate.
 
Reproduction Steps
Set the dedicated server to DeathPenalty value 1. Possibly observe value 2 upon death and respawn.

This issue should have been fixed in one of the more recent patches, so I’ll go ahead and close this ticket for now.

If you encounter the problem again in the upcoming v3 release, please submit a new report and include the output log along with reproduction steps so we can continue investigating.
