Clear missions are bugged.

We are having exactly the same issue.
Due to a power outage, my PC shut down, and my server files got corrupted, mainly the main.ttw and main.ttw.bak.
After searching through the internet, I found a solution to replace these old files with new ones.
Luckily, I had the starting world as a backup, so I copied those files, and it fixed our issue.
Now we are having a problem with the clear zombie quest; after 2-3 failed tier 6 runs, we decided to do some tier one so that we could search the whole area to see if there were any zombies hidden or something. Still, tier one was also not completed.
Then, I found a solution on Reddit that a restart might solve the issue. Luckily, it did.
But now, we have to restart the server every time to complete any clear zombie mission.
The fun part is if we do the quest alone, there is no issue whatsoever, but when we try to do the quest with the team, it won't clear.
Did you ever find a resolution to this? We're having this EXACT same issue. My cat turned my PC off, logged back in and files were corrupted. Fixed/replaced the same files you did, but now we're having to entirely do quests to get the completion. If we run it the first time, no success as it bugs. But rerunning the exact same quest the second time (without restarting) works. It's so weird.

To friends in the community... yesterday I went through this, I was in a group of 3 players and it went wrong, the mission was level 5 to clean the Pop-N-Pills factory, the door button wouldn't open the door, I opened it with the pickaxe and I killed the enemies, however, the mission kept warning me that there were no zombies in the room, after a lot of research, I found this forum and as an alternative solution I took the reward ticket to compensate, but, before deleting the mission, I had an idea, what if did I break the button? Well, I simply solved the bug, broke the button, reset the zombies in the room, then eliminated them and the mission was completed.

 
So had this bug today on a NDC checkpoint 4 Infested Clear.  Tried it 3 times.  The last group of zombies seems to trigger when I push the button on the "loot" room and step inside.  I went back through the POI and tried to "trigger" anything I missed - nothing happens.  I tried destroying the generators like one of the posts here suggested.  There's 3 generators in this POI - two small and one large.  I got one zombie to spawn destroying one of the small generators but thats it and that didn't complete the quest.  I finally gave up.  Appreciate all the loot but I'd like to complete the quest.

Still happening for me as well.  Single player.  I've only seen it happen with Grover High, usuallly a T6 quest.  In my current playthrough, it has happened twice.  The first time, I used the 'sleeper r' console command, which spawned a bunch of Zs, and then immediately used the 'killall' console command, which killed them all.  That finished the quest.

The second time, the commands worked as expected, but it never completed the quest.  So I had to resort to using the quest completion ticket, found in the creative menu.

Overall, this bug is much better than it used to be, but it does still exist.

Do you have the name of the POI by chance?

Thanks for the heads up.  We are aware of at least one reproduction to break that POI and working on a possible fix.

In your case, were you using any particular play style?  Did the orange navigation dot appear on any particular area in the POI?

I'm not sure what you mean by 'play style', but it's a strength build and at that point you have to use guns, as melee doesn't cut it.

In this case, I went to the roof to start the quest, where the Zs woke up.  I had to run them around the building a few times to kill them all as there are a LOT of them.  Once that was done, I went back up to the roof and made sure it was clear.  Then I went down the staircase and finished the clear, going along the intended path.

The orange dot, when this happens, is always up on the roof, usually around the center of it.

Just for reference, I tested Grover High on my game a while back when people were saying it was still broken.  I did it on a new save where I game myself the dev pistol so I could rush through it and see if there were problems.  I cleared everything in the normal order and did not leave the roof until everything was dead.  So this wasn't related to running around on the ground and them despawning.  With the dev pistol, everything was quickly killed.  After killing everything, I still had a yellow dot on the roof.  It was directly opposite from the ladder you climb up, next to the air handler unit that you can break into for hidden loot.  I think I posted a screenshot somewhere that showed the specific sleeper volume in the POI Editor.  I have a feeling that this is the same spot everyone is running into who can't compete Grover High.

 
There is a fix for the clear bug at Grover High.  Please keep in mind this is just one fix and there might be other fail conditions.

If anyone experiences another quest fail there in 1.3, please kindly submit a bug report with as many specific steps what led up to it.  Thank you!

I thought i was getting this just now at Crack-A-Book T5 clear. I had a single dot left on the roof but no mob there. However, i figured out what happened:

It turned out i had punted that mob over the side of the building (Stun Baton with Repulsor mod). I went back to the street level where it was waiting for me, and the clear completed at that point.

That last dot should probably attach to the mob in question in such a case ;)

 
Nevermind, i just had a similar issue on T5 Wasted Stiff's, Clear infestation:

This time there was no punting involved. The missing mob was just never there/never spawned/never triggered.

No orange dots at this point. Went down a few stories and:

Found some missing outside the building. However, 1 last dot left when they were done.

Did a quick sweep of each floor on the way down, and killed everything around the building at street level as well, no dice. Just the dot in the above screenshot showing.
