I've got all the underwear and an Advanced Insulated Armor mod in each piece of armor. And I'm still cold in the snow biome.

Because it's 7 Days to Die I can't hold the torch in one hand and do something else with the other hand.

What am I missing?

What levels? Clothing comes in 3 tiers, with 5 quality levels per tier. Are you using all quality 5 thermacore clothing? And what about the mods? What color are they? There are 3 tiers. Red, then Green, then Blue. Do you have blue mods in all armor?

In my game, I have blue in all armor and have two Q5 thermacore clothing items, one Q2 thermacore, and one Q3 thermacore. I am rarely cold in the snowy forest as long as I'm not wet. I am pretty sure that if I max out all the clothing, I won't be cold at all unless wet. Even when I get cold now, I'm only barely cold (48F or so, where anything over 50F is not cold, I believe). That's at temps at around 12F. I'm not sure how much lower the temps can get. And, again, this is when not wet.
 
The mods are all blue. The head, chest hands and feet undergarments are levels 3, 3,1 and 3 respectively. Is it true undergarments can't be crafted?

Not that I know of. I find it mostly in clothing piles and racks, so going to the clothing stores increases your chances. For higher level stuff, focus on the wasteland.
 
Yeah, that's why you're cold. You'll need higher tier stuff. That's only tier 1 out of 3 and low quality as well. You need to increase your game stage either by leveling or by looting higher tier biomes to find the better stuff.
