Character died while repairing a wall

I had something happen to me that I did not expect. I was repairing a wall when what looked like a chunk came out and then I died. I don't know why that happened. . In the next screenshot. I pulled the date.Could anyone tell me what happened? Please and thank y

i've never had this happen before 1.x versions, but have had it happen a few times in 1.1, maybe 1.0 as well; it surely has to be a bug
