PC Ready to spawn in the world ?

Why this question at the start of the game (?) if there are no answers to click?
However, in Italian the same question reads:
"Where do you want to respawn?"
It only stops the automatic start of the game.
I don't understand.

It means you can do something else while waiting for the game to load. For me, it's a nice QOL addition. I usually watch a stream on my other monitor (mute if it's a 7 Days stream), or I may do other things while the game loads, so knowing I don't have to sit and watch it load is nice. If you have a more powerful PC than I do, your load time may be faster, and then I can understand that it would hinder your entry into the game. 

 
Depending where you are when you load the game, you could be attacked shortly after joining.  By having that, even if you start the game loading and go do something, you don't have to worry about that.  It is especially helpful if the game is starting right before horde night starts.  Yes, it does mean you can't just start the game, leave, and when you return you are already fully loaded into the game.  But I think the benefits outweigh having to wait an extra 5-10 seconds to finish loading the last little bit into the game.

 
As you have to download the world the first time you join a game with a RWG map, it can take a few minutes. This is a nice addition so you can go afk meanwhile. I´t literally just a button so you don´t spawn in while afk.

 
Also, for modders, you can go into debug mode while on this screen, rather than waiting until you actually enter the game. This makes it easier to debug things that happen when the world first loads.

(To do this, press F1 and enter "dm".)

However, I agree that it would be good to have the option to not enter the game if you don't want to. (Quit to menu or just quit altogether.)
