Quest Completion Errors

So every time I make a new game or join one, the starter tutorial quests get a green check mark on each part completed. Later I do another quest like...Kill X # of Bears, etc. And get the green check marks for each part completed. A few saves, and load ups later, the check marks are now red X's and I still somehow have the quests done. Is this a bug or can it be manually fixed.

I know it's pretty insignificant...but hey it's just a little question.
