Crafted, Looted, and Quality

Heyas all! I've done some modding here and there for my own group's wants. I've got a couple of things I've been tasked with and thought I'd just see if I could get some advice.

The outcome is that we want a max of quality 3 items to obtainable in loot, traders or quest rewards.  I 'think' I can cover that in traders and loot xml.  Maybe the quests are somewhere other than the traders - I didn't look yet.

The other thing that seems to get more difficult is that we'd like quality 4 5 and 6 weapons, armor, tools to be only obtainable via crafting. In addition to that level 4 recipes should have fewer ingredients, 5 cost more, and level 6 items be ludicrous to give them that 'epic' feel once you are able to make it. Playing permadeath with destroying your gear on death is what we're after. 

That part seems a bit more complicated. I am not understanding how to differentiate between the same item at different qualities.

Anyway... hope this is worthy of some replies/advice.

You can set the quality levels on all that.

For both loot and quest rewards, look at the loot probability tables for them, they will be structured to show probability at each quality level.  Just replace those with new tables that max out at quality of 3.

 
I will find my old code and post it as an example.  I just recently got back from vacation so I can now be more detailed in my response.
