Zombies will move to the source of where the sound came from. So if you open a door and then sneak away they may not actually know where you are. Now, there are some issues with sounds in this game where if you throw something, like a molotov, the sound seems to come from you when it explodes...
Plenty of people mod items.xml all the time. There is more than one LBD mod out there right now. Just because you don't understand how to properly mod it is neither a bug nor an issue with the xml. There's a modding forum where you can ask for help if you are stuck on something.

Funny how many people complain about the dungeon POIs taking choice/exploration out of the matter, but as soon as a POI doesn't hold your hand all the way through people also complain.
If you're worried about mods breaking, you shouldn't ever be on the experimental builds.
Running an in progress build that can get updated at any time and break all mods and being surprised things broke. <------Dumb right?
